In order to process this application, we need:
- a first copy to close the meter at your old address
- a second copy to open the meter at your new address
- 1 copy of the identity card (front and back) of all persons concerned

Once you have completed the form, scan it and attach copies of the identity cards of all persons who signed the document. If you are a proxy, please also attach a copy of the mandate.
